Why do I need to see a Urologist?

https://youtu.be/QgGF3zf58es

If something malfunctions in your urinary tract — your body’s system for getting rid of your urine — you may need to see a special doctor called a urologist. They are experts who can treat problems that range from kidney stones to cancer.

What Is a Urologist?

Your urologist knows every function of the urinary system, which includes your kidneys, bladder, ureters (thin muscles that carry pee into your bladder), and urethra (the tube that drains pee out of your bladder.)

Urologists also treat a man’s reproductive system, which includes the penis, testes, scrotum, and prostate.

A urologist could even serve as your primary doctor if you have:

Regular problems with kidney stones
Prostate cancer
Long-term problems with holding your urine

What Kind of Training Does a Urologist Have?

Besides 4 years of medical school, urologists have at least 5 more years of special training focused on the urinary tract and male reproductive system.

Some urologists narrow their specialty to one area of care, such as things like cancer, women’s urology, children’s urology, male infertility, sexual health, kidney stones, or reconstructive urology.

When do I see a Urologist?

A urologist might treat bladder problems, urinary tract infections (UTIs), bladder and kidney cancer, kidney blockage, and kidney stones.

Men might also see urologists for:

Erectile dysfunction (ED)
Enlarged prostate gland
Prostate cancer
Testicular cancer

Women might also see a urologist for

Problems holding your pee after pregnancy
Pelvic organs prolapse (when an organ like the uterus or bladder starts to drop or press into or out of the vagina)
Children might need to see a urologist if they have an abnormal urinary tract problem like bedwetting.

Review 1 Perfect! Better experience than with any other hospital in Singapore.
Review 2 Probably the best hospital in Singapore. Family members were admitted here a few times. Doctors are great, nurses too
Review 3 Was there a weekend ago to accompany my relative who was admitted for 2 nights for observation and diagnostic tests. I must say that I was impressed right from the moment we stepped out of the lift.

The staff from concierge, clinic, ward, and even food court staff was friendly and helpful. The encounters throughout the 6 hours I spent there were pleasant, to say the least.

There was no doubt waiting time for admission but the wait was nothing that was inconvenient and inefficient. Commendable efforts are put into making the environment and experience as comfortable as possible for patients and visitors. At least it was for us.

P.s. There are limited carpark lots hence be prepared to pay extra for valet service unless you are willing and able to wait. Not the cheapest rate too, but it shouldn’t come as a surprise too 😉

Review 1 I was referred to Dr. James Tan by my insurance concierge and the first appointment was quickly set up within 3 days. My first review with Dr. James was insightful; he was very patient in explaining the procedure to me and at the same time reassuring me that I’ll be in good hands.

The night before the surgery, I (stupidly) read up on the procedure online and viewed some of the recorded videos of the operation. It freaked me out as this is the first time I’ll be doing a GA/LA, and also my first time getting cut open by the doctor while being in a sedated state.

The next morning, I requested to see Dr. James once more to confirm the type of procedure (also partly to see him again to calm my nerves) I’ll be going through and he reassured me again that it’s just a very minor operation and it will be over before I know it.

I must say I’m glad to have gone through this with Dr. James, and I’m very happy with the post-surgery results! Google reviews seem to be lacking for this small clinic; thought I should do my part to get people to know more about his clinic and give him a try.

A post-surgery appointment was set a week later and I was completely discharged.

All in all, the whole process (from scheduling my first appointment to discharge) took a little more than 2 weeks. Pleasant experience (but hopefully won’t have to visit again 😅)

Review 2 Super great urologist! Highly recommend!
Review 3 Dr. Gerald is very friendly. He can calm me down easily as I’m quite distressed with my condition. Then he explained to me thoroughly with detail and guided me on what to do next. Besides, the reception is also very friendly. Overall experience is happy.

Review 1 Spent 3 nights here after undergoing surgery. The main surgeon was based in Raffles while the anesthetist and one more consultant doctor were from other clinics. I guess they don’t have all the experts in this hospital. But overall the doctors and nurses were very nice, the food was good, the room was comfortable. Convenient location as it’s next to Bugis MRT. Bill was not cheap though!
Review 2 Great customer service here. Very efficient as well!!!
Review 3 Fantastic hospital! They’re so helpful and professional. I’ve never had such a helpful consultant appointment where the doctor really took time to run through everything with me clearly. They were able to also do blood tests within an hour and a half and have the results back for me straight away. I would highly recommend it.

The average Urologist visit: procedures and examinations

Your first appointment with a urologist may not be different from a visit with your general physician.

You fill out forms and answer questions about your health history, current symptoms, and any medicines you take.

The urologist will do a physical exam that includes a genital and rectal exam.

They may also order blood work or imaging tests, like a CT scan or ultrasound, for a closer look at your organs.
Here are the typical things you can expect during an appointment with a urologist:

A urologist will request you to provide a urine specimen, so do not go to the office with an empty bladder.

Many urological conditions may prevent you from being able to hold urine in.

Hence, once you arrive, inform the office staff that you are ready to provide a specimen.
Each urologist’s office visit starts with assorted paperwork.

This may include questionnaires to help assess how severe your ailment is.

On the questionnaires, you to “rate” things like lower urinary tract symptoms, incontinence, and/or sexual health.

You may also complete a “voiding diary” to documents times and amounts of urination if (and when) you experience incontinence.

You will enter an exam room and a staff member will record your detailed medical history.

It will concentrate on your genitourinary system and what your underlying problem is as well as involve a complete review of all body organ status.

The disease of other systems can help diagnose urologic problems.

Be prepared to provide a complete list of all your medications, including over-the-counter drugs, vitamins, and supplements that you consume.

If you think you may not recall the names of everything you imbibe, carry all your medications along.

The urologist will conduct a physical exam.

They will concentrate on the genitourinary system, evaluate other organs too.

The physician will perform a genital exam plus a digital rectal exam to assess the prostate.

After the examination, the urologist will discuss a treatment plan for you to determine your status.

This will usually involve additional tests, either during this visit, or alternatively, at a later visit.

The urologist may want to check blood counts, kidney function, or test PSA (prostate-specific antigen) or testosterone levels.

Your urologist may request imaging studies.

This may include sonography of the kidneys, the bladder, and/or the prostate; or an imaging scan to visualize specific organs.

The urologist may recommend an ambulatory, office-based procedure.

It may be cystoscopy, a minimally invasive procedure that examines the bladder and urethra; urodynamics, which assesses bladder functionality for patients who have incontinence; a biopsy.

A focused, detailed urologist office visit will enable doctors to determine, learn the correct course of action to treat your symptoms.

Trends in Urology in Singapore

The Singaporean public is now more aware of urological diseases such as prostate and testicular cancers, thanks to the more frequent conducting of public awareness talks.

The Internet has also made the search for medical information easier.

Relative to the past, men are also more willing to come forward when they have issues in the bedroom.

Now, they know that these conditions are potentially treatable.

They also know where to get treatment legitimately instead of relying on non-prescribed potentially fatal drugs from back alleys.

With regards to the treatment in general, most patients are not just looking for disease control alone, but also consider the possible side effects as well as risks.

In the treatment of urological disease, there are potential consequences for sexual function.

This is because the urinary tract is intimately related to the genitalia, especially for men.

Sample costs of Urology in Singapore

As per Singapore’s Ministry of Health (MOH), here are some prices for the commonly performed bladder scope procedure.

The prices are based on past transaction fees, so this serves only as a guide. Do note that bladder scopes are typically performed in the clinic or as a day surgery procedure.

Bladder scope (with or without biopsy)

Public, unsubsidized: $626 – $889

Private Day Surgery: $1,826 – $3,270

Private Clinic: $1,284 – $1,953

Do note that for private hospitals, 25% of patients are charged lower than the lower bound and 75% of patients are charged lower than the upper bound.

Public hospitals get subvention from MOH, so their price ranges would seem lower than those of private hospitals.

Urologist Consultations: Features, Checks, Tests, Costs, Functions

Urologists work to diagnose and treat, male or female reproductive systems, especially the Kidneys, Adrenal Glands, Urethra, Bladder, or Reproductive Organs. These alternatively undergo training to perform surgical or medical treatment of ailments that afflict these organs.

If you must consult a urologist, you might discover that prices range very widely, over a large spectrum, based on diverse, variable factors.

Without insurance, expect to pay bills from lower charges of $270 to high charges of $741+ to access basic 1st-time consultations with urologist doctors, national median charges are $350. If you are an advanced patient, then the total bill might be ½ of this. Such bills, nonetheless, often exclude many alternative services, on-demand, inclusive of lab checks or therapy. Total bills vary according to the form of consultation, the number of visits, checks, specific doctors consulted, or clinic location.

Dr. James G. Knight, MD, for instance, publishes all bill charges on his website. The regular consultation charges are $246, while occasional price discounts might lower total bills down to $123 to cover a consultation. Alternative treatments, published, listed on his website, could vary from a low charge of $4 for urinalysis or reach as high as $4,584+ for more advanced prostate surgeries.

Urologists Consultation: Regular Features

Over a consultation, the urologist may very often demand a urine sample prior to consultations, or akin to a regular doctor’s visit, one often is required to fill up detailed particulars about ailments to inform the clinic staff of personal medical details, disease symptoms.

During regular checks, a urologist might closely examine your genitourinary system or assess additional organs to analyze the symptoms. Consulted urologist doctors might then conduct a genital check or digital rectal checks to examine prostate health. Following the check, based on the urologist’s final diagnoses, these may negotiate specific diagnostic treatment methods for diagnosing the sources of medical symptoms. These might involve some blood count checks, PSA checks, or some imaging scans, that may all not get factored in bill drafts. Such checks may, then, vary, range widely based on diagnostic results.

Additional costs, supplementary charges, extra bills

All charges just quoted apply just to beginning consultations, exclude additional types of supplementary checks, feature merely as partial portions of total bills in case you suffer from symptoms, disorders. The bills may exclude factoring prescribed scans, medications, surgeries, or laboratory checks; so, in case further, checks are needed, this may charge hundreds, perhaps even thousands of dollars to all bill estimates quoted. Regular checks usually conducted over a consultation could cover a cystoscopy, urinalysis, biopsy, or uroflowmetry test.

In certain cases, a urologist might prescribe a cystoscopy to check the urethra and bladder. These checks are usually performed in a 3rd-party ambulatory section just outside the urologist clinic.

Ways to reduce medical bills

Speak to the urologist with a call prior to appointments to negotiate all possible options for treatment methods. Many clinics are willing to provide final bill estimates over phone inquiries or consultations, so even if you are not worried about high expenses, heavy bills, it could prove wise to closely examine the clinic’s reviews, performance, or affiliations to public or private hospitals.

Supposing one holds medical insurance, these should usually extend full coverage to most treatments, surgeries that the urologist performs for medical needs. Regularly consult your health insurance agent far in advance to ensure that the urologist you consult will accept your insurance policy subscription.

In case you lack medical insurance coverage, you may usually request the Urologist to grant discounts through footing bills in cash or via credit card payments. Many regular clinics will accept these, which will allow negotiations to final medical bills.
The urinary tract works to enable body drainage. It holds in, puts together, or releases urine. If certain complications develop, this leads to discomfort and humiliation, create much hassle, trouble. This may lead to greater, severe medical complications.

In case one contracts urinary tract issues, this is common:

  • Around 34% of adults of age 20 or older experienced a minimum of 1 urinary tract infection or some bladder infections.
  • Incontinence (Urine leakage) afflicts around 35% of females aged 65 or older or 10% of women below 65 years.
  • 20–46% of men of age 40–69 experience medium-to-full erectile dysfunction (failed attempts to achieve and sustain erection status).

Many urologists tend to have diverse, varied types of the urinary tract or urogenital disorders, with zero referral usually required. However, it could be prudent to consult a GP, polyclinic physician initially. Following this, one may exclude other alternative causes, sources of medical disorders.

Suitable, proper treatment may ease ailment disorders.

Symptoms or Therapy

Urologists attend to these conditions:

  • Kidney and urologic stone disease
  • Incontinence
  • Kidney cancer
  • Prostate cancer
  • Enlarged, noncancerous prostate gland
  • Erectile dysfunction
  • Problem urinating
  • Kidney blockage
  • Cancer of the urinary tract
  • Blockages in 1 or 2 tubes which transport urine over from kidneys over to bladder

Urologist services include:

  • Minimally Invasive Surgery (Surgery which involves Small Incisions, occasionally employing automated assistance of robotic machines)
  • Cryotherapy (utilizing freezing temperatures to treat some medical symptoms)
  • Vasectomy (certain types of male reproductive control)
  • Greenlight PVP Prostatectomy (Surgeries for eliminating excessive prostate cells)
  • Penile Prosthetics (an inserted rod that enables the penis to get firm)

Before the consultation

Preceding the consultation, a clinic nurse aide could request your health particulars. This could then make the urologist consultation a more fruitful, constructive visit for you. Often you are asked to answer these:

  • Cystoscopy (allows doctors to examine the inner urinary tract)
  • Urethroscopy (allows doctors to investigate the ureters or the kidneys)
  • Removing Kidney Stones
  • Inserting Stents
  • Removing the lesions found on testicles or scrotum

Prostate ailments may cause such symptoms in males, however, the cause for females is likely to come from some bladder, ureteral, or kidney malfunction origins. Doctors specializing in Urology undergo training in treating medical ailments afflicting the Urinary Tract (encompassing kidney, bladder, or prostate cancer), generally termed urologists.

Based on research reports published in The Journal of Clinical Oncology comparing the effects of urology treatment on urologic cancer deaths, having access to urologist doctors within certain specific American geographic locations eventually led to decreased deaths in urologic cancers within these geographic locations. Urologist doctors treat patients suffering a large spectrum of urology ailments, with many clinics in many American towns, states.

Gender-Specific Risks for Bladder or More Urological Cancers

The American Cancer Society found that Bladder Cancer features as the 4th highest recurring cancer with males (whereas females have 3 times lower likelihood than males to contract bladder cancer). Nonetheless, kidney cancer features as the 10 highest incidence types of cancer afflicting both men and women. Urological cancers alternatively may occur in greater incidence in individuals of 50 years, or above.

Furthermore, medical research reports published from 2019 made certain correlations connecting these benign urological symptoms with developing urological cancer:

  • Hypospadias
  • Peyronie’s Disease
  • Male infertility
  • Cryptorchidism

Benign Prostate Disorders, Prostate Cancer Incidence in Males

Gradual aging or gradual onset of active androgens remain the sole possible risks related to Benign Prostatic Hyperplasia (BPH) – that gets very frequently termed generally as the enlarged prostate. As observed by the National Institute of Diabetes or Digestive and Kidney Diseases (NIDDK), the BPH generally afflicts about 50 percent of males aged 51-60, or about 90 percent of males aged 80 or above. (Prostate cancer is also a high incidence diagnosis for males aged 50 or above.)

Getting high PSA examination results could indicate prostate cancer, however, this may not always confirm that tested males have prostate cancer. Though some positive biopsy reports are often regular ways to confirm prostate cancer diagnoses, gradual cancer onset is often 20 years and up reaching to terminal phases – also, zero treatments could be required for elderly males that get confirmed prostate cancer diagnoses.

Interstitial Cystitis with Females

Interstitial cystitis trends more commonly with females rather than males, generally afflicting about 6 percent of female populations within the USA (reported by Interstitial Cystitis Association). General symptoms are a constant urge for urinating, pressured bladder sensations (or bladder aches), or intermittent pressing pelvic aches for females afflicted. Many urologist clinics observe these could bring about long-term, perennial effects upon a female’s quality of living or often get discovered with females aged 30 or more.

Treatments could encompass many (perhaps all) of these symptoms

  • Medications directly inserted into bladder valves
  • Physical therapy
  • Oral medications
  • Surgery
  • Stimulating nerves

Ways in which Urologists finalize treatment regimes

An initial consultation with a urologist starts gradual steps to determine the origins or optimum medical treatments for diagnosed urological symptoms. Often, the urologist starts by reviewing a patient’s complete health records, with a focus on correlations to old urological symptoms.

Subsequently, diagnostic checks (inclusive of urine or blood examinations, or scans) get regularly undertaken. Upon results of these diagnostic tests become released or the diagnosis eventually is confirmed, the patient’s medical treatment regimens may then get determined.
Urology refers to surgical specialization which closely examines the urinary tracts of men or women or the reproductive organs of men.

SingHealth, via public hospitals, delivers full treatment to address the alleviation of urological disorders.

Regular Treatments

Treatments to alleviate Prostate Cancer

Prostate cancer refers to malignant tumors which regularly originate from external linings of the prostate organ. This can spread over to internal sections of prostate organs or out from the prostate to more sections in the body. These often remain treatable upon early detection but might lead to death with late diagnosis, improper treatments.

Robotic-enabled Laparoscopic Radical Prostatectomy (otherwise termed as da Vinci Robotic Surgery) trends as a major forward advance for surgery. The minimally invasive treatment is deployed for advanced, complex surgery needed for urological disorders inclusive of prostatectomy, taking out of prostate gland organs for prostate cancer.

Stress Urinary Incontinence: Medical Treatments

Stress urinary incontinence refers to spontaneous leakage of urine upon one coughing, sneezing, laughing, running, or lifting heavy objects. This originates from low-energy pelvic floor tissue worsened by childbirth, menopause, or pregnancy. These feature as highly prevalent types of urinary incontinence with females, while just over 10 – 15% of females contract these symptoms. Females suffering such ailments may not just be affected financially, physically, socially, but mentally, work performance, or family welfare, might then suffer problems after this.

Urinary Stones: Medical Treatments

Calculi, more frequently bladder stones, are compact solid crystals found within fluids in bodily releases, waste that includes urine. These are often located within the kidney (gall stones), or the pathway for urine over from the kidney organ to the urinary bladder, or Ureter (Ureteral Stones).

Men’s Ageing Treatments

Though more attention is given to menopause for females, men’s aging features as ailments that remain regularly ignored. Testosterone hormones for men usually drop gradually following the age of 30, with the rates of 1-2% yearly. Following this phenomenon, males over 50 might contract andropause, manifested in symptoms inclusive of low physical energy, lower libido, erectile dysfunction, low motivation levels, or sleep disorders. These could then suffer fatigue, low mood, or impaired mental capacity. Specialists from Changi General Hospital now specifically opened the Andropause or Men’s Health Clinic to specially treat men suffering from aging issues.

When should one consult a urologist?

Supposing you exhibit 1 or more of the symptoms below, you need to seek some medical advice:

  • Blood, or traces of blood within urine fluids
  • Difficulty releasing urine
  • Pain passing urine
  • Backaches from growing kidney stones
  • Difficulty with conceiving (or the intent to check if causes are reproductive issues with the male)
  • Difficulty performing in bed generally

These are not exhaustive issues listed, however, make a formal appointment to see a urologist upon facing any such difficulties, issues.

Urology tests: Distinct types of checks

Uroflowmetry (UF)

Uroflowmetry refers to some highly specialized checks performed by most urology clinics. Termed regularly as uroflow checks, these trends as regular diagnostic tools employed for assessing the speed of urine flow.

The urologist might prescribe some uroflow checks in case one suffers the following symptoms:

  • Low urine levels
  • Slow urination
  • Urine flow issues

Urine test results may potentially provide the consulted urologist with estimates of whether are any potential blockages within one’s urethra or one’s prostate, or whether there are high urine levels left in bladders following urology patients completing all passing of urine flow.

Occasionally urology tests might indicate if bladder organs are “weak”, with which contractions of one’s bladder upon voiding fails, falters. In case results require more investigation or affirmation, some urodynamics studies could get undertaken.

Urodynamics study (UDS)

The UDS refers to more detailed or invasive studies. This could check the:

  • urinary bladder performance in relation to keeping urine
  • voiding urine
  • urinary continence
  • further interruptions of bladder functions, performance
  • speed of urine flow

Ultrasound Scans

Ultrasound Scans often are undertaken within some outpatient medical clinics. Hence, these may assist with faster, better diagnoses of medical issues.

Scans resemble, seem like supplements to the clinical examination, enabling doctors to investigate bodies closely. CT Scans or MRI Scans may get requested when the physician senses that these are needed.

Cystoscopy

Cystoscopy, alternatively termed as the bladder scope, entails pushing an adjustable scope holding scanning camera over at edges up one’s urinary tract. Following which, the doctor may then gain access to view:

  • prostate (for the men)
  • bladder
  • urethra

Specific treatments that urologists perform

These specialized urology treatments:

  • Laparoscopic Surgeries (keyhole) with and without robot devices aiding
    Open Surgery
  • Microsurgical Surgery
  • Endoscopic Surgery (employing scopes pushed up the urinary tract)
  • Extracorporeal Shockwave Procedures (giving shocks to the physical body e.g., ESWL for stone treatments, ESWT for treatments of chronic pelvic pains, or erectile dysfunction)

MOH Pricing

According to Singapore’s Ministry of Health (MOH), the following are prices for many regularly undertaken bladder scope treatments.

These prices were sourced from old transaction rates, so these services are just as price estimates. Take note that bladder scopes usually are undertaken in medical clinics or as basic day surgery treatments.

Take note that for private clinics, 25% of patients get charged below the lower bound rates, while 75% of patients get charged lower than upper bound rates.

Public hospitals receive subventions from MOH; thus, their price variations might seem lower than charges at private clinics.

It remains recommended that one confirms with or clarify with urologists about rates because there are multiple considerations. It remains highly crucial to take heed that rates remain highly variable according to considerations that include:

  • complexity of ailments
  • expertise or training of engaged physicians
  • where urology treatments are performed

What can one expect for an initial consultation with a urologist?

1st consultation:

If you are consulting a urologist, you must go holding a full bladder. Usually, the urologist might request a urine sample to test. This applies particularly in case you consult urologists to check for urinary ailments.

To add, the urologist might check medical records or undertake some medical tests. Certain urologist physicians might further ask for ultrasound scans within the consultation cubicles to help with final diagnoses of urinary ailments.

Blood tests, urine tests, or further additional specialized scans inclusive of X-rays, CT scans, or MRI might further get requested if needed.

It remains important to cooperate fully with engaged urologists to treat medical issues because these might enable the urologist to prescribe suitable treatment solutions. This may further facilitate prescribing an ideal treatment, some feasible prescription options.

Conclusion

One must undertake consultation with a urologist in the case that one:

  • Experiences pain urinating
  • Sees blood inside one’s urine
  • Has sexual difficulties — like difficulties conceiving or performing

Urologist: A broad definition

A urologist physician specializes in diagnosing or treating ailments of one’s urinary tract or male reproductive organs.

Urologists: Job scope, Duties

As earlier mentioned, the urologist attends to multiple, diverse, urology symptoms. These are some disorders of the urinary tract or urinary system:

  • stone diseases — stones found in kidneys, bladder, ureters
  • cancers — prostate cancer, kidney cancer, bladder cancer, ureteric cancer
  • infections — cystitis, prostatitis, pyelonephritis
  • urinary tract
  • urinary disorders owing to an enlarged prostate, that include benign prostatic hypertrophy
  • neurogenic bladder
  • overactive bladder

To add, these further treat symptoms that afflict the male reproductive organs, encompassing, but not restricted to:

  • male genitalia cancers —penile cancer, testicular cancer
  • sexual dysfunction — erectile dysfunction, ejaculatory disorders
  • male fertility problems
  • disorders of the penis — Peyronie’s disease, congenital phimosis, penile curvatures, and taut foreskin

Are urologists differentiated by gender basis?

Urologists may treat patients of all sexes. But several urologist doctors do hold extra sub-specialties. Here are certain sub-specialties which are gender-based:

Female Urology

These do not indicate that a urologist is female. Those classed in this sub-specialty treat urinary symptoms that occur in women patients.

Hence, these are usually more skilled, trained to treat certain functional urology symptoms that include voiding dysfunction.

Andrology

Andrologists specialize in treating male patients with:

  • Subfertility
  • voiding disorders — symptoms of benign prostatic hyperplasia (BPH)
  • sexual dysfunction

Certain andrologists may act as prosthetic urologists. Such physicians are experts in penile implants or urethral slings. These insert implants or slings for treating:

  • penile curvatures
  • male urinary incontinence
  • erectile dysfunction

Further specialties

To add, there are more specialties which generally cover:

  • Urinary stone disease
  • Uro-oncology (cancers of urinary tract organs or male genitalia)
  • Renal transplants
  • Reconstructive Urology

Supposing you are uncertain about which sub-specialist, you should consult to treat ailments, you could see a general urologist, that may be capable of reviewing your symptoms, or refer suitable physicians.
