The process of fat grafting 

The fat grafting process includes three main steps: extracting fat from a body part using liposuction, processing the fat, and reinjection of the purified fat into the body part that needs enhancement. 

The first step

The first step involves using liposuction techniques to extract fat from one body part. The results are best achieved by manual methods such as thin liposuction cannulas with small holes. Other liposuction methods such as laser and ultrasonic liposuction should be avoided because they may destroy fat cells. 

The second step

The fat cells are washed with saline solution to remove fat cells which did not survive in the fat grafting. These fat cells may even pose problems to the fat cells in the new transferred area. 

The third step

The fat is reinjected in tiny droplets throughout the area of treatment. The amount of fat injected varies according to each patient’s specifics and the body part of the treatment area. 

Post fat grafting care

Fat grafting is a day surgery procedure, and you will be able to return home on the same day of the surgery. After you gain consciousness after the surgery, you will begin to experience a mild discomfort and aching over the fat donor body part. This can be quickly resolved with prescribed painkillers. There might also be swelling and bruising at the donor body part. However, this is usually resolved after two weeks post-surgery. Patients should avoid exertion and raise the donor body part upright to 30 degrees for the fastest recovery. Patients should also avoid exerting pressure over the fat grafted area to ensure maximum fat cells survival. 

Types of anaesthesia used during fat grafting 

There is a wide variety of anaesthesia used for different body parts of fat grafting. The anaesthesia used depends on the volume of fat extracted from the excess fat body part using liposuction. Local anaesthesia is used to extract a small volume of fat as there is not much pain involved. The larger volume of fat (more than 500cc) always require patients to be sedated under general or IV anaesthesia.

Using your body fats from one body part to another, you can reduce excess fats from other parts of the body and restore the body area you are getting treated on. Additionally, the harvested fats are scientifically compatible with your fats, reducing any risks or complications that might occur from the body rejecting these tissues. 

Permanent results 

The results of fat grafting are permanent as long as patients maintain their current weight. The healthy fat cells which are transferred via fat grafting are living substance. During the first six weeks after the fat grafting session, a small portion of fat will be naturally absorbed by the body, while the fat cells that survive will live and thrive in their new location. The key to maintaining the fat grafting results is living a healthy lifestyle and staying active. 

Common areas of fat grafting 

Breasts

The most common body part for fat grafting is women’s breasts. There is usually a 25cc to 400cc volume of fat that can be injected into each breast. The patient can expect the breast to increase its size by one or two cup size. Some of the reasons why fat grafting to breasts is common are as follows: 

  • Corrects breast asymmetry and improves the shape of sagging breast
  • Repairs radiation tissue damage and treats breast implant capsular contractures
  • Correct scar deformities from lumpectomy or breast reconstruction

Scar

The face is also a common area of fat grafting among patients. Fat grafting can rejuvenate facial skin, giving more volume to deflated skin with natural-looking results. Fat grafting can help patients eliminate their laugh lines around the mouth, reduce the appearance of fine lines around the eye area, and plump up their cheeks or lips. 

Hands

The back of the hands is the most common sign of ageing. As individuals grow older, the padding at the back of the hand becomes thinner. Fat grafting is a more natural way to give the patient’s hands a more youthful appearance and rejuvenated look than fat filler injections. 

What is the survival rate of fat cells after fat grafting

When proper liposuction techniques are used, at least 60% of the injected fat cells will survive in the injected area. This would mean permanent results. There are several ways to improve the fat survival rate, such as:

  • Using low suction power to aspirate fat cells by hand, carefully 
  • Use centrifugation to separate the fat cells from the unwanted fluid
  • Inject the fat cells in multiple layers carefully 
  • Avoid over injection of fat cells, which will prevent the increase in tissue pressure and blood flow

Is fat grafting suitable for everyone

Unfortunately, fat grafting is not suitable for everyone. An eligible candidate for fat grafting needs to have sufficient fats to be harvested from the donor body parts: abdomen, knees, buttocks, thighs, and flanks.

As fat grafting is not suitable for everyone and can be a costly procedure, people interested in fat grafting should check for their eligibility.
